『METAL GEAR SOLID Ⅴ： THE PHANTOM PAIN』は スネークというコードネームを持つ男の半世紀にわたる戦いの歴史を描いた「METAL GEAR SAGA」において、シリーズ最高傑作であり集大成といえる作品です。
本作の持つ魅力のひとつは、オープンワールドの「メタルギア」＝時間経過・天候変化の概念のある広大なフィールドでの「自由潜入」です。
昨年発売した『METAL GEAR SOLID Ⅴ』のプロローグにあたる、『METAL GEAR SOLID Ⅴ： GROUND ZEROES』では、限られたフィールド、かつミッション毎に天候や時間も決まった条件下での潜入を体験頂けましたが、本作では『GROUND ZEROES』の200倍以上という広大なフィールドになり、ミッション内で刻々と時間も天候も変化します。ミッションをクリアする手段もルートもプレーヤー次第という、本当の「自由潜入」をお楽しみ頂けます。
【ストーリー】
ソ連のアフガニスタン侵攻以来、冷戦は新たな局面に移行していた。
1984年、隻眼に義手の男がそのアフガニスタンに現れる。
スネークと呼ばれるその男は、過去に米国の非政府諜報機関サイファーにより、歴史の表舞台から消された伝説の傭兵だった。
昏睡から目を覚ましたばかりの彼を襲撃者から救い、アフガンへと導いた工作員“オセロット”は言う。
スネークのかつての相棒カズヒラ・ミラーがソ連軍に囚われている。彼を単独で救出することで伝説の復活を世界へ示せ、と。
それは9年前、彼らの仲間を惨殺したサイファーへの復讐へと続く、世界を股にかけた戦いの始まりだった――。
『グラウンド・ゼロズ』を経て、“V”本篇がここに結実。

フィールドでは、兵士や捕虜、車両、兵器、資源、動植物など様々なものを フルトンというバルーンで自分の拠点となるマザーベースへ回収することが可能です。マザーベースでは、武器やアイテムの開発、ミッション中の弾薬やアイテム、武器の補給といった、ミッションを遂行する上での支援を行う事ができ、様々なものを回収することで、マザーベースの機能を拡張・拡大することができます。
【ミッションのサポートとしてバディの同行が可能に】
ミッションをサポートしてくれる相棒（バディ）として、ゲームに登場するほかのキャラクターを同行させることもできます。先遣として潜入させたり、自身の攻撃のサポートとして狙撃させたりすることで、プレーの戦略の幅も広がります。
